Assisted Living Waiver Program (ALWP)

The Assisted Living Waiver Program (ALWP) is a comprehensive program designed to facilitate a secure, seamless transition for Medi-Cal eligible seniors and persons with disabilities from a nursing facility to a community-like setting in a Residential Care Facilities for the Elderly (RCFE).

The ALWP program not only aids individuals in transitioning from nursing facilities but also serves eligible seniors and persons with disabilities already residing in the community who are at risk of being institutionalized. The ALWP services can be used to create a program tailored to safely meet their care needs, allowing them to continue living in a Residential Care Facilities for the Elderly (RCFE).

Eligibility Criteria

Please note that individuals should have full Medi-Cal benefits. Individuals who have a share of cost, spend down, or other deterrents to their Medi-Cal must address these issues before or during their application process.

Pre-Screening Process

The pre-screening process is the initial step toward the ALWP program. Here, applicants can complete an intake application for pre-qualification. Our office will carefully review your application and inform you of your enrollment status.

Enrollment Process

Once your pre-screening application is accepted, we will move forward with the enrollment process. Our office will schedule an in-person assessment with a registered nurse (RN) to determine the required level of care. This process is an integral part of the enrollment as it allows us to understand your specific care needs. We will then assist you in gathering the additional required documents to complete the admission approval process.

Apply - Pre-Eligibility

To initiate the process, complete and submit the application packet for review of pre-eligibility criteria. Upon meeting these criteria, Sierra Care Coordination Agency will facilitate your enrollment into ALWP at no expense. Rest assured, our team will guide you through every step of the enrollment process seamlessly.

Waitlist Process

If you're at home or in assisted living, you'll join the DHCS Waitlist. After submitting your application, we'll send your Waitlist Request to DHCS. Our wait time may vary depending on DHCS. Once off the Waitlist, we'll schedule your Initial Assessment. Immediate assessment may apply if hospitalized or in a Skilled Nursing Facility for 60+ days.

Assessment

An RN Care Coordinator will perform an in-person or virtual assessment to gauge your Medi-Cal ALWP Eligibility, employing the DHCS Assessment Tool. This comprehensive tool will assess all your care requirements thoroughly. Upon eligibility confirmation, our team will review and submit the complete assessment to DHCS for approval.

Review & Approval

DHCS may take up to 6 to 8 weeks to approve the assessment. Upon approval, we have 180 days from the initial submission date to secure placement at an ALWP Approved Facility. Rest assured, we'll diligently work to facilitate your transition within this timeframe, ensuring a smooth and timely process.

Secure ALWP Transition

With a selection of over 800 ALWP approved facilities in Southern California, our team will collaborate with you and your loved ones to secure placement at a facility that meets your care requirements.

Ongoing Care Oversight

As the CCA, we'll monitor participants' care throughout enrollment. An RN Care Coordinator or social worker will conduct monthly visits to ensure they receive adequate care at the facility. This ongoing oversight ensures participants' needs are consistently met.
